Another part of our registration process is to submit paperwork that is either required by the Kinnelon Public School District or the NJSIAA.  

Hard Copies of the Health History and Doctor's Physical Form must be handed in to your Coach.

By signing your electronic signature above,  we consent to random testing in accordance with the NJSIAA steroid testing policy. We understand that if the student or the student’s team qualifies for a state championship tournament or state championship competition, the student may be subject to testing for banned substances.

 
 



 
KHS STUDENT ATHLETIC/ACTIVITY AGREEMENT
 

 We have viewed the 2013-2014 Student-Athlete/ Club and Activity and Parent Handbook.  We have read and understand the rules and guidelines set forth in this handbook.  By signing this form, we agree to participate under these guidelines.  

We also agree to participate in the Random Drug Testing Program at Kinnelon High School.  

As a student-athlete or a member of a club/activity, I pledge to not participate in the illegal consumption of alcohol, CDS (controlled dangerous substance) and to not use tobacco while a student at Kinnelon High School.

I also understand that if I am charged with possession or constructive possession of any of these substances or found under the influence of any of these substances, that I am in violation of the Athletic/Activity and Kinnelon Board of Education Substance Abuse Policy.  

If I violate this policy during my time at Kinnelon High School, the consequences outlined in this document will be implemented.  The policy begins from the time I play (first practice) my first sport at Kinnelon High School or attend my second club/activity meeting until my graduation.  Consequences for violating this policy are progressive and carryover from sport to sport or club to club and from grade 9 until graduation.
 
 

 
ATHLETIC DEPARTMENT QUESTIONS AND CONCERNS PROCESS

Kinnelon High School Athletics values the commitment to using athletics to be an extension of the classroom.  To that end, every student athlete is encouraged to take personal responsibility for their own concerns and questions about their sport and athletics. As such, we further encourage all student athletes to interact with the coach at appropriate times if there are questions or concerns about their play.  

Coaches are required to conduct a Pre-Season Parent Meeting and are encouraged to attend booster club meetings or find other ways to interact with parents.  During the pre-season, NJSIAA regulations allow for one player only meeting to distribute paperwork.  Coaches are also encouraged to meet with athletes throughout the year to explain decisions that impact their individual performance and methods and suggestions for improvement.  
 
Bearing in mind that our Athletic Department desires strong communication with both the student athlete and parent, we have created a generalized process for questions and concerns.  The process is as follows: 

1)  In the event that questions or concerns arise the student athlete must first meet with the coach to discuss the question or concern.  The meeting should be requested for a time that is either before or after practice.  Moreover, this meeting shall not be held during an academic period of the school day. 

2) In the event that the student athlete and coach are unable to resolve the issue, the student athlete, parent, and coach may arrange a meeting to be held during the working hours of the coach. 

3) If this meeting does not lead to a resolution then the group may arrange a meeting with the Assistant Principal - Athletics. 

4) The next step in the process would be to request a meeting with the Building Principal. 

5) The final level for questions or concerns would involve the Superintendent of Schools.  

The Administration recognizes that there are situations that will cause a different approach as well as a deviation from the aforementioned process.  The Assistant Principal - Athletics will be the primary point of contact if a student athlete or parent has a situation that needs to deviate from the process.  

Additionally, coaches and administrators will not have open discussions about playing time or in game coaching decisions/strategies.  These situations are decisions that are determined by the Head Coach and also involve other student athletes.  When meeting with any level for a question or concern, only the individual student athlete will be discussed.

Coaches will not be available to meet with parents immediately before or after athletic contests.  In addition, the Administration and Coaches will not meet with parents during an athletic contest.  These are emotional times and often the concern or issue is amplified by the tension of the moment.  It is strongly encouraged to utilize the 24 hour rule; meaning waiting 24 hours prior to requesting a meeting or contacting the coach.

KINNELON PUBLIC SCHOOLS CONSENT TO RANDOM DRUG TESTING FORM

 
I understand fully that my performance as a pupil and the reputation of my school are dependent, in part, on my conduct as an individual. I hereby agree to accept and abide by the standards, rules and regulations set forth by the Kinnelon School District Board of Education and the sponsors for the activity in which I participate

I authorize the Kinnelon School District to conduct a test on urine which I provide on-site to test for alcohol and/or drug use if my name is drawn from the random pool. Pursuant to the Regulations for the Pupil Random Drug and Alcohol Testing Policy, I also authorize the release of information concerning the results of such tests to designated District personnel.
